Patient Paperwork

Nicotine use, whether from cigarettes, vapes, cigars, gums, e-cigarettes, patches or smokeless tobacco can increase the risk of complications and slow down healing after surgery. Nicotine can cause your body to work harder, and it can interfere with blood flow, oxygen levels and the immune system. Patients must be nicotine-free for eight weeks prior to scheduling an appointment. Please call to schedule eight weeks past your nicotine-quit date. 

We ask patients complete the new patient paperwork* before your initial consultation.

*When completing paperwork, we advise using a desktop computer rather than a cell phone or tablet for saving and uploading your documents.
